The team achieved its improvement through engaging the whole department, improving workflow, holding blood pressure clinics and installing clear protocols for the medical assistants, effectively using their diabetic roster reviews with an integral role for the clinical pharmacists, and having a highly engaged population management coordinator.
The Chelmsford Quality Management Team includes:
- Melvin Nemkov, MD, Site Medical Director, of Weston
- Kim Schortmann, Internal Medicine Operations Manager, of Grafton
- Carolyn LaRosa, Population Management Coordinator, of Chelmsford
- Nathan Samuels, DNP, of Salem
- Juliemae Grace, Clinical Support Supervisor for Internal Medicine, of Dracut
- Margaret O’Leary, Clinical Practice Supervisor of Internal Medicine, of Tewksbury
- Diane Cote, PharmD, of Woburn
- Margi Larsen-Byrnes, Site Administrator, of Lincoln
The Clinical Innovation Award recognizes a clinician or clinical team for their extraordinary efforts contributing to clinical innovation within a Harvard Vanguard practice.
